Good Faith Estimate Disclosure
We currently provide individual therapy to Illinois and Indiana residents only. If you reside outside of these states and are seeking wellness services, please schedule a consultation to determine if you qualify for other services.
Good question. Yes, depending on the clinician, we are in-network with the following insurance companies:
Blue Cross Blue Shield (IL and IN)
United Healthcare (IL and IN)
Aetna (IL and IN)
We recommend calling your insurance company directly to confirm your in-network coverage including co-payment amount, deductible etc. Self-pay/ Out of network investment rate is between $80-$200/session.
As the expert of your lived experience, if you are willing to do the hard work during the session and continue your healing process between sessions Courageous Spaces is a good fit for you. Our therapist specializes in specific issues such as stress, anxiety, relationships, parenting, depression, trauma, family conflicts, grief, religion, and self-esteem.
Courageous Spaces is not a good fit for you if:
You are in an immediate crisis or an emergency circumstance.
The courts or any other authority required you to undergo therapy.
Yes, everyone can benefit from therapy! We believe therapy is not limited to you having a “diagnosis or problem”. Whether you need to process something ‘big’ or ‘small’, we are here to support and meet your needs.
After you schedule your free 15 min discovery consultation, and it is determined that you and your therapist are a good fit, you will receive an email with next steps information for scheduling a formal date and time for your initial intake session.
For your initial intake appointment, you will be sent an email invitation to enroll in your secured client portal to sign your electronic intake paperwork. The intake paperwork must be completed within 24hrs of your first session. The portal will allow you to meet with your therapist via telehealth sessions and sign as well as review ongoing clinical documentation.
During the first appointment an intake session would be conducted which consists of a diagnostic evaluation, completing a depression and anxiety assessment, reviewing Courageous Spaces policies and payment information, and scheduling your future appointments
At Courageous Spaces we specialize in creating a safe and affirming space for caregivers and parents who are raising children and teens with neurodiversity who are underserved in wellness spaces; however we provide services to all demographics dealing with a variety of emotional and mental health issues. Under the law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
Make sure your healthcare provider gives you a Good Faith Estimate in writing at least one business day before your psychotherapy service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
